Welcome to Heart of The Ozarks

Heart of the Ozarks Healthcare Center, built in 1968, is a Skilled Nursing Facility nestled in the beautiful Ozark Mountains of Douglas County, Missouri. We are a beautiful 120 bed facility that sits atop a hill overlooking the city of Ava. We provide both long-term and short-term stays. We accept Medicare, Medicaid, private pay, and most insurances. We are proud to provide 5 star, 24-hour skilled nursing care to our residents. Our staff is truly committed to providing high quality patient-centered care in a home-like setting. We have an exceptional dietary department which is proud to provide our residents with a wide variety of menu options.
